Personal attendance of the applicant, accompanied by parents or legal guardian.

Original copy of the Colombian Birth Certificate.

Two (2) 2.5X3.0 cm color photographs facing the camera with a white or blue background

Rh. factor and blood type.

Copy of the birth certificate.

Rh. factor and blood type.

In order to correct the information relating to incorrect dates, place of birth and/or identity number a duplicate may be requested by fulfilling the following requirements:

Original identity card (element to be corrected).

Two (2) full-face photographs in color with white or blue background, size 2.5X3.0 cm.

Original birth certificate with the corresponding notation.

Rh. factor and blood type.

Colombians over age 7 and under 18 living abroad who have lost or damaged their Minors’ Identity Card may request a duplicate at a Colombian consulate through their parents or legal guardian by fulfilling the following requirements.

Personal attendance.

Provide the number of the identity card.

Two (2) full-face photographs in color with white or blue background, size 2.5X3.0 cm.

Original copy of the birth certificate.

Rh. factor and blood type.
