A name can be changed once only and the following are the requirements for the purpose:
- Present the valid citizenship card.
- Photocopy of both sides of the citizenship card.
- Authentic copy of the Birth Certificate (issued no more than three months previously).
When the deed has been drawn up, you must:
- Forward a copy of the deed to Colombia for your Birth Certificate Civil Registration to be annotated.
- Request a copy of the Birth Certificate with the annotation.
- Rectifiy your citizenship card.
- Finally, change the name on your Colombian passport.
