• Original death certificate issued by a foreign official or entity certifying the date of the death and the name and sex of the deceased.
• Original citizenship card of the deceased and his/her Colombian passport, if any, and a photocopy of both.
• Original citizenship card or passport of the person reporting the death and a photocopy of same.

Please note that documents issued by another state which are in a language other than Spanish, must be translated by an official translator and have an apostille or be legalized, as the case may be.

Please also note that, if the registration is to be made before the Colombian consulate of the circumscription where the marriage took place, the consul can authenticate or effect recognition of the documents without the need for either apostille or legalization.

• Personal attendance of the informant
• The informant must present his/her original citizenship card and a photocopy of same.
• One (1) original copy of the Marriage Certificate or Registration issued no more than three (3) months previously.
• One (1) original copy of the Birth Certificate of the Colombian bride or groom.
• One (1) original copy of the identity document of each partner, which must be a citizenship card for Colombians and a valid passport for foreigners.

• Presence of the person to be registered must accompany the informant.
• Original of the Live Birth Certificate or Civil Registration of the person to be registered. The certificate or registration must be issued where the birth occurred. If you do not have a live birth certificate, you must have sworn statements by two witnesses who are aware of or were present at the birth.
• If the baby is more than one month old, one of the above documents or the baptism certificate must be presented.
• Identification of the parents: if they are Colombian, photocopies of their original citizenship cards on separate sheets of paper. If either of the parents is of a different nationality, his/her valid passport and a copy of same must be presented.
• Certificate of blood type and RH factor of the person to be registered.

Requirements

A person who requires a copy of a civil registration of a birth, marriage or death registered or reported abroad must request it from the Colombian consulate where the death was registered or reported.

• The serial number of the registration requested and the date.
